PROFESSIONAL SERVICES AGREEMENT

Welcome
Our professional relationship aims to provide an empathic, non-judgmental and confidential space in which you can explore the issues that matter to you. The work is collaborative. I will listen carefully, seek to understand your experience and, where appropriate, offer reflection, questions or challenge. I cannot promise a particular outcome, but I will work within the limits of my competence and in accordance with applicable professional and ethical standards.
Please read this agreement and the attached Contract Notes and let me know if you have any questions.
You may confirm your agreement by signing below, by email, or verbally during a session.
Sessions and fees
Sessions normally last 50 minutes and may take place online or in person, as agreed. Our work will usually be open-ended unless we agree a fixed number of sessions. We will review the work periodically, including whether it remains helpful and whether any changes are needed.
Current standard fees are: Online sessions: £90; In-person sessions: £110
Fees are normally reviewed each January. I will give reasonable advance notice of any increase.
Payment should be made in advance unless another arrangement has been agreed. Payment information is provided on invoices and in the Contract Notes.
Cancellations
If you need to change or cancel a session, please give at least seven days’ notice. Where less notice is given, the full fee will normally remain payable because the appointment has been reserved for you. I will try to offer another suitable time within the same week where possible and may exercise discretion in exceptional circumstances. No fee is payable where I cancel a session. Any advance payment will be credited or refunded.
Communication
Outside sessions, communication will normally be by email or text and should usually relate to appointments or practical administration. Emails and messages are not monitored continuously, and an immediate response cannot be guaranteed. Sentia Well-being is not an emergency or crisis service. Please see the Contract Notes for urgent-support information.
Registration, appointments, records and reminders are managed through Halaxy. Invoices may be issued through Tide bank and/or GoCardless.
Confidentiality
Information shared during our work, and the records relating to it, are treated as confidential.
Confidentiality may be limited where:

you have given consent;

I reasonably believe there is a serious risk of significant harm to you or another person;

safeguarding action is necessary;
-
disclosure is required by law, a court or another lawful authority;

information is reasonably required to establish, exercise or defend legal rights.
Where it is safe and lawful to do so, I will normally try to discuss a proposed disclosure with you first and will disclose no more information than is reasonably necessary.
As a Registered Member of the British Association for Counselling and Psychotherapy (BACP), I undertake clinical supervision. Relevant aspects of the work may be discussed with my supervisor. Identifying information is minimised and pseudonymised wherever reasonably possible, and supervision is itself confidential.
Agreement
The attached Contract Notes form part of this agreement. By signing below, confirming acceptance by email or giving verbal agreement, you confirm that you have received this agreement and the Contract Notes; had an opportunity to read and consider them; had an opportunity to ask questions; agreed to work together on these terms.

Where agreement is given by email or verbally, Jeremy Gordon will retain a dated record of that agreement.
Contract Notes
These notes form part of the Professional Services Agreement.
Nature of the service
The service may be described as counselling, psychotherapy, coaching or psychological support, depending on the nature of the work, what we agree, your location and any relevant legal, regulatory, professional or insurance considerations.
For international clients, the service may be provided as coaching or psychological support rather than psychotherapy where appropriate.
This service is not a substitute for medical care, psychiatric treatment or emergency support. I do not prescribe medication or provide medical diagnoses.
Online sessions
For online sessions, please join from a private and reasonably suitable location and take reasonable steps to protect your confidentiality. If the connection is lost, I will first try to reconnect, then send a text and, if necessary, telephone you. I may ask where you are physically located during an online session so that appropriate support could be contacted in an emergency.
Emergencies and urgent support
Sentia Well-being is not an emergency service, and emails and telephone messages are not continuously monitored. If you or another person may be in immediate danger, contact the emergency services available where you are located.
If I am seriously concerned about your safety and cannot contact you, I may contact your emergency contact, GP, healthcare provider or emergency services.
Records and privacy
Sentia Well-being Ltd. is registered with the Information Commissioner’s Office. Your information is processed in accordance with the Sentia Well-being Privacy and Data Protection Notice, available at:
[Insert direct privacy-notice link]. Adult counselling and psychotherapy records are normally retained securely for seven years after the professional relationship ends. The Privacy Notice explains what information is held, how it is used and protected, who it may be shared with, your rights and how to raise a data-protection complaint.
Recordings and automated tools
Neither of us may record a session, or use automated transcription, artificial-intelligence note-taking or similar tools during a session, without the other person’s prior explicit agreement.
Social media
To protect confidentiality and professional boundaries, I will not intentionally follow, follow back, look up or communicate with clients through social media. You are free to view or follow public professional content, but visible interaction may reveal your connection with me to others. Please use email or another agreed method for communication about our work.
Payments
Invoices may be sent by email through Tide bank, GoCardless or another payment service. Payments may be made by bank transfer, payment link or agreed payment mandate.

Endings and interruptions
Either of us may raise the possibility of ending or pausing the work. You may end at any time. Where possible, I encourage you to let me know so that we can discuss the decision and arrange an ending session. If I need to pause or end the work, I will normally discuss this with you, give reasonable notice and consider alternative support where appropriate. Immediate action may sometimes be necessary because of safety, serious boundary concerns, legal or professional requirements, illness or other exceptional circumstances. If I cancel a session, no fee will be payable, and I will offer an alternative appointment where reasonably possible.

Governing law
This agreement is governed by the law of England and Wales, subject to any mandatory rights that apply where a client is located elsewhere.
